One "hung POST" fix.

Fixed graceful restarts when mod_backhand is statically compiled into Apache.

Fix Connection: Keep-Alive header multiplicity.

Backed out the TABLES_MERGE.. Wrong solution that breaks cookies.
Implemented a method similar to the util_script.c in Apache.

Don't unset content-length on connection: close requests.  It confuses the
progress bar on people's browsers.

Keep-alives should be enabled if it is HTTP/1.1.

Fixed "losing cookies" problem.  TABLES_SET should be TABLES_MERGE.

BackhandProxied: removed from r->headers_in on proxying host.

Fixed outstanding "incorrect $REMOTE_ADDR" bug.  It was caused by the r->connection->remote_addr being already munged from the first request on all subsequent requests over a single HTTP connection.  This triggered when the servers allow keep alives.  A "cleanup" function was registered in the logger callback to remedy this.
